Additional names I need your help with.

The following is part of a list I received recently of additional members we have lost. I need your help if you recognize any of these names please email me with any information you can supply as I only have a week to go and can't find anything on these. I have them listed by the state they were located in. On the list of those I have already found, it seems that some names may be more than two years old but I have not already recorded. I don't have their exact date of passing which is causing some problem in locating any information as well. If anything I at least would like to know what chapter they belonged to.
